Approach
You are at the center of all we do.
We use a model called “Patient-Centered Medical Home” to guide our service. It means your primary care provider heads up a team of nurses, health educators, pharmacists and others (like specialists or therapists) who work together to keep you healthy, by using evidence-based care.
You are the centerpiece of the team — the most important member.
- Get to know you: We will learn about you, your family, and life situation. We will keep accurate records and update them every time we are in contact. We will listen to you. You are a full partner in your care.
- Communicate with you: Listen to your questions and concerns, and answer them in a way you understand. We are happy to explain your health situation clearly and make sure you know all the options for care. We want to help you make the best decisions for your care.
- Support you: We will help you plan for your care, set goals, and help you meet them. We offer information about classes, support groups, and other resources to help you stay healthy. We refer you to other trusted experts when necessary.
In this approach, we expect you to take charge of your health, be active in your own care, and talk with us about any changes, concerns, and challenges. If you receive care from any other health providers, let us know so we can coordinate the best care possible.
