Community- and School-based Services
KidsCARE Community and School-based Services brings whole-person care directly to schools, early learning centers, and childcare centers in your community.
KidsCARE provides mobile pediatric and dental services, and in some locations, onsite school-based health clinics. This means you can get your kids’ health care needs tended to while they’re in childcare or at school. We also offer telehealth visits, and when necessary, in-person visits for your child at our Swope Health Pediatric Clinic or at a Swope Health satellite clinic.
Swope Health KidsCARE Community and school-based Services offers:
- Medical and dental evaluations and exams needed for school enrollment and to maintain your child’s health. This includes routine well-child care, immunizations, screenings for hearing and vision, lead testing, general health assessments, and routine dental exams and treatment.
- Individual healthcare plans for your child’s chronic conditions or special needs such as asthma, food allergies, and diet restrictions. Some schools require annual plans for the management of medical conditions.
- Referrals to a pediatric specialist, if needed.
- Referrals for behavioral health services, if needed.
- Assistance in obtaining health insurance (Medicaid and CHIP).
Who can participate
Swope Health KidsCARE offers an Early Childhood (Pre-K) School-Based Health program (birth to 5 years) and services for students in grades K-12 at partnering schools and early learning centers.
Ask your child’s school, childcare or early learning center if they are a Swope Health KidsCARE partnering school or center. If they are, the school nurse, health coordinator or family advocate will assist you in enrolling your child for our KidsCARE programs.
There is no fee to register. When your child comes in for services, your child’s insurance will be billed. We offer financial assistance to those without insurance based on rates adjusted to income (and those rates may not require a co-pay). We also have specialists who can assist with Medicaid enrollment if you qualify.
Also, anyone can register for full services online, or at any Swope Health clinic.
