Patients should know…
Social determinants of health, such as the inability to pay or lack of health insurance coverage, can hinder many individual’s ability to obtain healthcare. However, Swope Health’s premise is to break down these barriers.
I have been given the gift of education and knowledge and I believe it is my duty to use these productively to make a difference in the lives of people who need the most help.
As your primary care provider, I can offer you healing, health promotion, and education and I am dedicated to providing you with the best high quality evidenced-based care. In turn, it is my hope to improve patient outcomes and inevitably, the health of the community. As my patient, it is my goal to provide you with an individualized care plan, continuity of care, and health literacy to help with the shared decision-making process.
